Do NOT complete this form unless you, your family member, or friend are fully READY to surrender the Cavalier.

Once this form is submitted, if you are in Arizona, we will likely have transport available within 24–72 hours. Surrenders outside of Arizona may take longer.

Unlike most rescues, we do our best to provide updates throughout the process because we know how emotional this decision can be.

We only accept Cavalier King Charles Spaniels (no mixes).
If surrendering a Cavapoo, breeder paperwork or a DNA test confirming the dog is a Cavapoo is required. We cannot accept unidentified poodle mixes.

Please complete the form honestly and in detail. We do not judge and we are experienced with medical and behavioral needs.

A signed release of ownership is required before transfer. Your Cavalier will be loved, protected, and carefully placed in a cherished forever home.
